In trying to get rid of your facial skin flakes, you might be over-cleaning your face. You might want to try something like L’oreal go360Clean deep facial cleanser for Sensitive skin. Don’t get the one for oily skin–I think that you are trapped in a never-ending cycle of stripping your skin of moisture and then your skin pumps out more oil to compensate.
The product I recommend has a little soft scrub pad that will exfoliate the flakes, but the formula is gentle. I would say that you should only clean your face once a day. Then after your pat your face dry use a little moisturizer–try Cera Ve. (both of these products are not super high-end.)
Make sure you drink plenty of water.
Stop using all of the other products and again, only wash your face once a day. After about a week, your face should start recovering, then you can start washing it twice–once in the morning and once before bed. I think you are trying very hard, but you may actually be causing this unending cycle. Best of luck to you. (The masks and toners are stripping all of the oils out of your skin resulting in dry top layers, then your face gets oilier. Gentle cleansing and exfoliation and a lotion like cera ve is much better than cold cream which is like grease. The 360 will gently take off the flakes, the Cera ve will hydrate your skin–it is also non-comedonic. So, try both of my suggestions and dump the cold cream. Exfoliation, not toners or masks, Cera-ve. Try it for a week before you decide it won’t work.)
